The Integrated Care Coordinator provides end-to-end, non-clinical case coordination for participants in TFDSOF’s Health Initiatives Program. This role manages intake through closeout, coordinates referrals, tracks budgets, and ensures accurate, timely Salesforce documentation while serving as a key point of support for Special Operations families.
